Life’s transitions can be complex—but you don’t have to manage them alone. Anglicare’s Guidance and Support services connect you with the right programs to coordinate your NDIS plan, find new living arrangements, and access support quickly.
We take a person‑centred approach to help you grow confidence, achieve your goals, and live more independently.
Our Support Coordinators help you understand your NDIS plan, connect with trusted service providers, and make the most of your funding. We simplify the process and help you build the skills to coordinate your plan independently over time.
If you’re looking to live more independently, we can help you design a personalised ILO arrangement. Working with you and your circle of support, we’ll develop a flexible living setup that reflects your preferences, strengths, and goals, whether that’s living alone, with housemates, or with shared support.
Through practical and compassionate case management, we provide ongoing support to help you overcome challenges, access essential services, and maintain stability. Our team ensures you’re connected with the right people and resources to achieve positive, sustainable outcomes.
Community Assistance and Temporary Support (CATS)
The Community Assistance and Temporary Supports Program (CATS) supports people living in the ACT who have been discharged from the hospital and require home and community care due to daily challenges arising from a health condition.
Referrals to the CATS Program can be made by clinical hospital staff, general practitioners, community support organisations, family or carers, as well as through self-referral.
When a person contacts either a CATS Service Provider or the Central Intake Service (CIS), an initial screening is conducted to determine eligibility. If eligible, the screening process will also assess the person’s needs and determine whether prioritisation and/or wait listing is appropriate.
Additionally, the screening process should evaluate whether a different program might be more appropriate.
To refer yourself or someone else to the ACT Government’s Community Assistance and Temporary Support (CATS) Program, contact the Central Intake Service on 02 5124 6500.
The phone line is open the following days (except public holidays): Monday, Tuesday, Thursday, Friday: 8am -5pm and Wednesday: 8am- 3:30pm
